At Alcedo Care, we understand the diagnosis and symptoms of a mental health disorder can significantly impact an individual’s life. Alcedo Care offers specialised nurse-led care dedicated to providing exceptional mental health care at home and in the community.

We can support people of all ages in a variety of settings, including  individuals under various legal frameworks including Community Treatment Orders (CTO), community court of protection orders (including DoLs), care orders, parental consent as well as informal care arrangements.

What does mental health home care service include?

Our mental health support services range from hourly 1:1 support through to high levels of enhanced observations 24 hours a day.

To ensure our clients receive the best quality of care, we provide a Registered Mental Health Nurse (RMN) overview to support:

  • Individual and bespoke 24-hour packages of care
  • Ad hoc support visits offering light touch support and supervision
  • Crisis Management and relapse prevention
  • Breakdown in care (adults, children and young people) including hospitals or other environments

We use a personalised and holistic approach to care, and our model is trauma-informed with the least restrictive practice at the heart of our care.

Conditions that we support

Our team have experience working with many different conditions including:

  • Long-term complex mental health conditions
  • Food and substance-related mental health conditions
  • Personality and mood-related conditions including schizophrenia, bipolar disorder, psychosis, EUPD (emotionally unstable personality disorder), etc.

Our complex mental health team support people of all ages, including adults, young people and children. We use a gentle approach (known as the low arousal approach) which focuses on reducing anxiety and promoting positive behaviours. We collaborate closely with the client, their parents or families, support staff, nurses and other specialists to create a positive and supportive package of care which meets the complex needs of our clients.

Our complex mental health service is offered in a variety of settings wherever our clients call home which may include:

  • Individual’s homes
  • Residential settings including secure residential services
  • Supported living services
  • Hospital environments
  • Regulated and unregulated children’s services

Our Approach

Our clinically-led mental health teams use a trauma-informed, person-centred, multi-disciplinary approach to wrap around the individual.

  • Clinical Coordination: Our Registered Mental Health Nurses (RMNs) coordinate seamlessly with local authorities, NHS, and community social care teams. This ensures we provide the best possible care to help individuals return to, and remain within, their communities with a better quality of life.
  • Specialist Expertise: We are skilled at supporting a wide range of complex mental health conditions, including schizophrenia, BPD, behavioural disorders and food and substance-related mental health conditions.
  • Seamless Integration of Care: We can address both complex physical and mental health needs simultaneously and ‘in-house.’ Our Registered Nurses and clinical teams – including Positive Behaviour Support (PBS) and Crisis Prevention (CPI) specialists – work together seamlessly.
  • Wider Network: Where required, we integrate associate and partner specialists into our packages of care to support highly specialist needs. These experts can form a core part of our wider Multi-Disciplinary Team (MDT).

Flexible and Holistic Clinical Approach

We understand that supporting individuals with complex needs requires adaptive, flexible care that can change day-to-day and week-to-week.

Our core focus is to stabilise and wrap around the individual to help them remain and achieve a good quality of life within their community, whether through wider social care teams or SEN school attendance.

Key elements of our approach include:

  • Integrated Multi-Disciplinary Team (MDT): Our clinical and mental health teams work flexibly and holistically.
  • In-House Specialism: We ensure therapeutic support is built-in, including expertise in managing behaviours of concern (Positive Behaviour Support / Crisis Prevention Institute – PBS/CPI).
  • Clinical Oversight: Provided by our Registered Mental Health Nurses (RMNs).
  • Behaviour Analysis: Our UK-SBA Behaviour Analyst develops functional behavioural assessments and up-to-date PBS plans, ensuring our care planning is effective and person-centred to current needs and environments.

Specialist Partner Network

We maintain an active and wider network of associate and partner specialists (including Psychology, SaLT, OT) that can be accessed and built into our packages of care whenever a specific specialist need or condition requires support. If necessary, these external experts can form part of our core or wider MDT.

Our experienced RMN’s bring a wealth of knowledge from diverse settings, including high-pressure environments such as acute care units and NHS trusts. They are skilled in a range of areas essential for providing comprehensive mental health support, such as:

  • Risk assessment and management: Identifying and mitigating potential risks to ensure the safety of clients and staff.
  • Psychosocial interventions: Employing evidence-based therapies to address emotional and psychological challenges.
  • Managing behaviours of concern: Developing effective strategies to de-escalate situations and promote positive behaviours.
  • Case management: Coordinating and overseeing comprehensive care plans to optimise outcomes.
  • Training and development: Continuously enhancing their skills and knowledge to stay up to date with the latest best practices.

Our in-house PBS Lead and CPI trainer practitioners, provide direct 121 support and practice leadership to our embedded network of area based Mental Health Workers. They actively support and oversee behaviour assessments and PBS plans to ensure behaviour incidents can be reduced and quality of life is supported.

To further ensure the highest quality of care, our mental health support workers undergo rigorous training tailored to the specific needs of our clients. Our highly skilled nurses develop bespoke training plans to equip support workers with the necessary skills and knowledge to provide effective support.

Crisis and intensive community based multi-disciplinary care services

Rapid Response Team: We offer a rapid response service for urgent placements

Intensive Community Care Service- ICCS: A specialist community-based multi-disciplinary service that provides early intervention, crisis prevention and intensive outreach for children, and adults with learning disabilities (LD) and/or autism, complex mental health, who display or behaviours that challenge. The service is designed to prevent unnecessary Tier 4 hospital admission or placement breakdown and effective intensive stepdown to community care by providing flexible, proactive and trauma-informed support to enable people to live safely and meaningfully in their community.
